Adoption Amend an Illinois birth record for a person born and adopted in Illinois Once the adoption is completed, you will need to submit: A certified copy of either the Illinois Certificate of Adoption (see FORMS in the right hand-hand column) or the decree/judgment of adoption.

The Vital Records office of the Illinois Department of Public Health has copies of births from 1916 to present day. Anything before 1916 must be obtained from the county clerk's office in the county where the birth occurred.
